Our delivery aims to overcome silo working and make connections between programmes and place. We recognise the need for work to be taken forward at a local level, to work together as a system on good practice and shared solutions and provide an opportunity to collectively shape and influence regional and national agendas and other key initiatives.
We achieve this by bringing together senior leaders of primary and community care and place representatives from across West Yorkshire and Harrogate in the format of a Primary and Community Service Programme Board. The Programme Board members oversee our programme of work and provide support and a direction of travel to achieve the vision and aims outlined in our strategy.
Complementary to this, the Programme Board works collaboratively with specific forums /groups including West Yorkshire and Harrogate Primary Care Leads Group. The group aims to:
- Deliver the co commissioning of primary medical services,
- Support NHS England & Improvement in the delivery of the commissioning wider primary care and
- Progress the delivery of the Programme Boards areas of work and priorities.
The Programme Board is also supported in its delivery of its Workforce agenda by the West Yorkshire and Harrogate Primary and Community Care Workforce Steering Group and six place based groups (Leeds, Airedale & Bradford, Calderdale, Harrogate, Kirklees and Wakefield).
Our programme is represented on other Health and Care Partnership programmes such as Capital and Estates, Digital and Interoperability and Urgent and Emergency Care and continues to build links and rapport with the wider Health and Care Partnership programmes of work for example, unpaid carers programme, Improving Population Health and personalisation and retains associate membership of other programmes such as maternity and prevention at scale.
The delivery arrangements for the primary care programme allows for place and Primary Care Network level priorities to feed into the Health and Care Partnership system, where there is opportunity to make connections between places and other programmes as well as shape and influence national agendas.
