The West Yorkshire Advance Care Planning and Bereavement toolkit includes resources for people to have better conversations, support and personalised care about advance care planning and bereavement.
The toolkit was developed to ensure our workforce is equipped with resources to connect and support people across West Yorkshire with a range of needs and backgrounds, who may be experiencing inequalities and barriers in accessing information, services and advocacy. For example, people with learning disabilities, people with dementia, people from different cultural backgrounds, and people experiencing homelessness.
Each resource was co-produced by staff from Overgate, Wakefield and St Gemma’s hospices working in partnership with a range of stakeholders and people with lived experience of palliative and end of life care.
We hope this toolkit will be used alongside the ‘My Future Wishes’ resources to ensure people throughout West Yorkshire have better personalised care, and shared decision making conversations about advance care planning and bereavement.
The toolkit also includes advance care planning and bereavement resources which have been published by other organisations.
