The West Yorkshire Continuous Glucose Monitoring (CGM) Implementation Plan Position Statement provides a strategic update on our 5-year roll out plan for CGM (and hybrid closed loop (HCL)). It aims to address concerns and refocus on the cost-effectiveness of this healthcare intervention, while ensuing a structured and controlled continuation of the technology rollout empowering people with diabetes and improving health outcomes.
2. Continuous glucose monitoring (CGM)
Continuous glucose monitoring (CGM) is be available on the NHS to anyone with type 1 diabetes in West Yorkshire. Some people with type 2 diabetes may also be offered a CGM to help manage diabetes.
Children and young people will usually be offered CGM.

4. West Yorkshire Continuous Glucose Monitoring (CGM) Pathway for Primary Care
This is a West Yorkshire CGM pathway for primary care - drawn up to support the implementation of CGM for those eligible with type 2 diabetes (and those with type 1 diabetes choosing to have their diabetes care and support provided through primary care rather than specialist diabetes teams in secondary care). Author of the pathway, Dr Waqas Tahir, WY ICS Diabetes Clinical Lead, said:
"This pathway aims to guide best practice by providing a consistent, evidence-based approach for clinicians. It consolidates essential guidance into a single document, supporting healthcare professionals in making informed, autonomous decisions when initiating eligible individuals on the technology. By following this pathway, clinicians can ensure that their actions are aligned with the highest standards of care, promoting safety, efficacy and patient-centred outcomes."

This communication about the Wakefield CGM implementation strategy is specific to Wakefield only. It is designed to align the implementation of NICE guidance for both type 1 and type 2 diabetes in Wakefield Place with the WYCGM commission policy and implementation strategy. b. Decision support tool: making a decision about managing type 1 diabetes
The new decision support tool has been designed to support people to understand what is available in the NHS, providing a resource to support discussions between someone living with type 1 diabetes and their healthcare professional about appropriate steps in their care, which they are eligible for and come to a shared decision about the person’s care in managing type 1 diabetes.
