T2DAY

The Type 2 Diabetes in the Young (T2Day) programme is an initiative for people with early onset type 2 diabetes. Under the programme, patients benefit from:

extra one-to-one reviews.
The option of new medicines and treatments where indicated, to help better manage their diabetes.
Dedicated support for women - during pregnancy, access to contraception and folic acid supplements.

Resources to support patients

Patient information pack (PIP)

We have partnered with Diabetes UK (DUK) to produced a patient information pack (PIP) which contains helpful information to support patients to live well with type 2 diabetes. The PIP provides advice on reducing the complications associated with diabetes. Download the PIP here and share with your patients.

Youngtype2 App (by Digibete)

West Yorkshire ICB now have access to the free DigiBete Youngtype2 App which provides those with early onset type 2 diabetes (EOT2D) clinically approved support, engagement and resources which are digitally accessible and therefore can be accessed whenever needed. The DigiBete Youngtype2 App helps those living with EOT2D understand the importance of their diabetes self care and management. However, the App does not replace the care given by an individuals diabetes team.

Find below a patient letter and other resources to help you explain to patients how the Youngtype2 app could help them. These resources will also be available in SystmOne and EMIS to enable sharing via text/upload to patient letters. Click the links below to view, download and share them with your patients.
