About the Improving Population Health Function
Our vision is to embed a culture of improving population health and reducing inequalities through increasing system capacity, capability and intelligence. Working closely with a wide range of partners across health and social care we aim to:
- improve health outcomes for all people by contributing to data, intelligence and insight and embedding a population health approach across our partnership
- tackle inequalities in experience, outcome and access of care by increasing capacity and capability across the system
- help support broader social and economic development by improving the core determinants of health through key partnerships
- to enhance productivity and value for money by embedding a culture of stewardship

We have shared roles with the West Yorkshire Violence Reduction Partnership and West Yorkshire Combined Authority and work closely with other teams across the Partnership. This includes the Mental Health, Learning Disabilities and Autism Programme. Partnership working across Bradford District and Craven, Calderdale, Kirklees, Leeds and Wakefield District is fundamental to the success of our programme. Coming together around common aims allows the programme to use the power of a collective voice. This helps to ensure that population health themes are featured throughout all we do as a partnership.

We have launched a campaign encouraging people across West Yorkshire to sign up as Suicide Prevention Champions and join the effort to reduce the area’s above-average suicide rate.
