A key priority for West Yorkshire Health and Care Partnership is supporting people with lived experience to build the knowledge, skills, and confidence to make meaningful impact.

Working with people and communities must be at the heart of designing and delivering future-proof health and care services. To embed personalised care across all services, we need to listen to and work with people with lived experience. This approach ensures care is truly personalised.

The Peer Leadership Development Programme, hosted by NHS England’s Community Health Services Team is a free programme that helps people living with (or caring for those with) long-term conditions develop personally and gain the tools to contribute effectively.

Delivered entirely online, the programme is co-designed with people with lived experience. It’s broken into easy-to-follow steps, combining self-directed learning and virtual group sessions.

Completing the programme’s three steps opens up opportunities to join the Peer Leader Network, connecting participants nationwide to share experiences and collaborate. It also provides a chance to shape and influence health and care services at local, regional, or national levels.

The Wakefield District Health and Care Partnership embraced the programme to establish a local co-production group, now called Stronger Together. This group meets monthly in person, bringing together individuals with diverse lived experiences who are committed to driving continuous improvement for their communities.
