A couple of weeks after I had been at the memory service, my wife received a call asking if the Care Coordinator from my GP surgery could come visit me to chat about my recent diagnosis.
(The care coordinator) entered my home, and made me aware, this was not a clinical visit, but for me to talk if I wanted to. Time to talk about how I was feeling and how I felt about my news. That was music to my ears. Somebody had come from my doctors to listen to me without a time limit.

What we are doing
- Introduced 4-6 week initial face to face Care Coordinator appointment after recent diagnosis for every patient within our Primary Care Network living with Dementia
- Every unpaid carer to have a carers contingency plan in place.
- Every patient will receive a personalised care and support plan which will be reviewed yearly, including those in a Care Home.
- Every patient and family member have support from a Care Coordinator based at their GP Practice.
- Completing Dementia Reviews annually.
