Supporting people to live well with diabetes

We have produced an infographic showing what we are doing to prevent and support people to live well with diabetes in West Yorkshire. Highlights include:

  • Referring 4,338 people to the Healthier You NHS Diabetes Programme (NDPP) from January to 1 April. The NDPP is a health coach supported programme that helps people at risk of type 2 diabetes to prevent or delay the condition through lifestyle changes
  • Consistently exceeding national monthly referral targets to the NHS Type 2 Diabetes Path to Remission Programme (T2DR). Since February 2024 we have referred 282 people to the programme which involves a total diet replacement approach shown to help some people with diabetes achieve and maintain remission
  • 1,845 people with type 2 diabetes accepting a referral to Healthy Living, a free online service designed to provide advice and information to help better manage the condition. Collaborative working across West Yorkshire, including with GP IT and data quality teams to roll our bulk text messaging has helped boost referrals
  • National Diabetes Audit data from 2023/24 shows we are continuously improving attainment against NICE recommended treatment and care for people with type 1 and type 2 diabetes

How are we doing to prevent and support people to live well with diabetes in West Yorkshire? NHS Diabetes Prevention Programme NDPP is for people with non-diabetic hyperglycaemia (elevated blood sugar levels); also known as prediabetes. This is a health coach supported intervention which prevents the condition progressing to type 2 diabetes. Avoiding diabetes prevents people developing a developing long-term condition and the many complications associated with these.  In West Yorkshire, in 2023-24 we exceeded the target set for us by NHSE to refer to Healthier You which means we are supporting more people to avoid diabetes.  4338 people have been referred since January 2024 to Healthier You, 3196 have started the programme and 12 people with a history of gestational diabetes have self -referred since 1st April 2024. NHS Diabetes type 2 path to remission programme The NHS T2DR 12 month programme involves a total diet replacement approach that has been shown to help some people with type 2 diabetes achieve and maintain non-diabetic glycaemic levels off all diabetes medication (commonly referred to as remission).  West Yorkshire is consistently exceeding the monthly referral target to T2DR set for us by NHSE. 282 people have been referred to T2DR since Feb 2024 - this referral number is significantly higher than for the same period in previous years.  In West Yorkshire we now can offer both a digital 1:1 and a new face to face 1:1 T2DR to individuals. It is important those not digitally enabled or who prefer face to face care are made aware of this new option.  Healthy Living programme for Type 2 1845 people with type 2 diabetes have accepted a referral to the Healthy Living programme; 1055 have started learning more about their condition and self management.  Collaborative working across West Yorkshire, including with GP IT teams and data quality teams, to roll out bulk text messaging to those eligible for Healthy Living lead to increased referral and programme uptake between December 2023 and March 2024, with numbers continuing to increase. Structured education for people living with diabetes is so very important - let's keep engagement levels high.  NICE recommended treatment and care for people with diabetes In West Yorkshire improving the attainment of the 3 treatment targets (3TTs) and 8 care processes (8CPs) for peopling living with type 1 and type 2 diabetes is a priority. Most recently published National Diabetes Audit data from 23/24 shows our attainment is continuously improving: Type 1 attainment 3TT’s 22.1% (England average 24.2%)  Type 2 attainment 3TT’s 35.6% (England 36.5% Type 1 8CP 26.1% (England 25.3%) Type 2 8CP 37.8% (England %)
