Purpose of the event: to engage a wider and diverse group of health, public health, social care, PWLE,and VCSE partners in conversation about personalised care, what it is why it’s important, celebrating what we are doing in WYH, how to get involved and what the next steps are.
Starting the culture change to a different way of working.Core message of the day – if we want self-care PC to really fly we need to change the culture, change the way we engage with people, the conversations we have with each other, the relationship between the clinician and the patient, the way services are commissioned.Big picture – culture change, what can I do.how do we change the culture
Aims:
- We (the primary care community and others leading transformation change in the system) understand the context of the WY&H HCP and the breadth of both challenge and opportunity for change
- We understand the breadth of expertise across the system and identify any gaps in knowledge or expertise
- Agree some practical next steps on the journey to establishing and delivering the personalised care agenda for the partnership – what should we ‘invest in’ and what would the ‘return’ be?
