Personalised care represents a new relationship between people, professionals, and the system. It happens when we make the most of the expertise, capacity and potential of people, families, and communities.
The NHS’ long-term plan says that more people will benefit from ‘social prescribing’, a personal health budget and new support for managing their own health in partnership with patients’ groups and the voluntary sector.
Evidence says that by involving people in decisions about their health and care we can:
- improve their health and wellbeing.
- improve their quality of care, and
- ensure they make more informed use of healthcare resources.
Personalised Care comprises of six key areas:
- Shared decision making – Working in partnership with the care team
- Enabling choice – Choosing from the available options
- Social Prescribing – Connecting to help and support in the local community
- Health Coaching and Supported Self-Management – Building knowledge and skills to manage medical condition
- Personalised Care and Support Planning – Designing own plan supported by professionals
- Personal Health Budgets and Integrated Personal Budgets – Managing own budget and support with help if chosen
Across West Yorkshire many of the elements of the personalised care model are already in. West Yorkshire was part of the NHS England Personalised Care Demonstrator Programme for three years to pump prime personalised care approaches across the West Yorkshire Health and Care Partnership. We have worked with support from NHS England to build, develop and spread the model of personalised care delivered locally in Bradford District and Craven, Calderdale, Kirklees, Leeds and Wakefield.
