Bradford Trident – Bradford

A one-year project to improve health outcomes for Eastern European, Gypsy, Roma and Traveller communities in Bradford.
Bradford Trident through the Roma Connect, will povide a range of activities designed to reduce health inequalities in the Roma communities through the support of trained and experienced staff working with individuals and families, either 1-2-1 or in small groups using a Life-Style Coach model.
Positive Action for Refugees and Asylum Seekers (PAFRAS) – Leeds
Amount funded - £25,000
A one-year project in Leeds to provide a triage service for service users a with a range of support needs.
Positive Action for Refugees and Asylum Seekers (PAFRAS) will use the NHS CT funding to support asylum seekers to access health care, with a specific focus on providing mental health interventions.
Open Country – Wakefield and Harrogate

A one-year project delivering outdoor activities for people aged 18 to 60 years old who have ongoing mental health issues, learning disabilities and autism across Wakefield and Harrogate. The Open Country project is aimed specifically at Open Country members with ongoing mental health issues, learning disabilities and autism.
Carers’ Resource – Leeds and Bradford

A two-year project to support young carers in Craven and Harrogate. Carers’ Resource will offer one-to-one support to young carers whose lives have been especially impacted by the pandemic, in schools in Harrogate and Craven districts to improve their mental health and wellbeing.
Touchstone – Leeds, Bradford, West Yorkshire and Harrogate

An 18-month project to deliver activities to address the perinatal mental health of women aged over 18 in Bradford. Touchstone through this project will provide specialist mental health outreach support to marginalized women living in Bradford experiencing low to moderate perinatal mental health issues, focusing on wellbeing and connecting with nature.
Women's Activity Centre (WAC) CIC Ltd – Halifax

A 2-year project to deliver activities to older women of South Asian origin living in Calderdale aimed at improving the health and well-being of this group and lowering hospital admissions.
The WAC based in Park Ward, Halifax will be re-opening to give older women a wide range of health-based activities and respite for their carers.

Basis Yorkshire – Leeds

A one-year project to increase access to healthcare services for women across Leeds who are sex workers and are sexually exploited. Basis Yorkshire through this project will address health inequalities and improving access to primary and secondary care health services for sex workers in Leeds, in partnership with Leeds Community Health Care (Health and Inclusion).
Bramley Elderly Action – Bradford

A two-year project to improve health and well-being outcomes for people aged 60 or over living in Bramley, Swinnow and parts of Stanningley (West Leeds). Bramley Elderly Action (BEA) is a community led health and wellbeing project for over 60’s, including assessment, one-to-one and group activity sessions. This will improve physical/mental health, happiness and confidence.
People Matters (West Yorkshire) – Bradford

A two-year project to deliver weekly participant-led wellbeing groups for people with learning disabilities from across Leeds and from the locality of Scholemoor in Bradford. People Matters will, through the project, engage and involve communities in activities to promote engagement in self-care management. Building and sharing expertise to address and remove barriers faced by disadvantaged adults when accessing health services.
Leeds Older People's Forum (LOPF) – Leeds

A two-year project to deliver an arts project for people over the age of 50 in targeted communities, focusing on the Halton Moor and Osmondthorpe, Richmond Hill, Holbeck and Seacroft areas of Leeds. Leeds Older People’s Forum (LOPF) through the Arts project will work with people (50+) in targeted communities, focusing on the Halton Moor and Osmondthorpe, Richmond Hill, Holbeck and Seacroft areas of Leeds, which are in the top 10% deprived in Leeds. The project will aim to build confidence and improve health and wellbeing using creative arts-based activity with a person-centred approach.
Inspired Neighbourhoods – Bradford

A one-year project to tackle mental health and multi-morbidities amongst individuals from the South Asian communities of Bradford, Wharfedale and Craven. Inspired Neighbourhoods will support adults primarily from areas of deprivation to access mental wellbeing support, tackling health inequalities. Due to Bradford demographics, our focus will be to support adults from BAME backgrounds, providing culturally and faith sensitive mental wellbeing services while tackling wider challenges of poverty, poor housing and debt. Our partner Equality Together will support adults with disabilities, Autism and LD as well as Welfare Advise.
BID Services – Leeds and Kirklees

An 18-month project to support those with a sensory impairment (SI), primarily aged 18 and over and their family and carers, including those who are hearing impaired (HI), visually impaired (VI) or dual sensory impaired. The project focuses on people living in the Leeds and Kirklees.
BID Services’ project addresses health inequalities people with sensory impairments face and breaks down barriers to health information/support by working in partnership with health providers across Leeds and Kirklees.
Huddersfield Mission – Leeds
Amount funded - £50,000
A two-year project to work with people from the Tolson area who access their services. This includes include people who are disproportionately affected by health inequalities such as those who are homeless, refugees and asylum seekers, the Traveller Community, and people with multiple and complex needs. Huddersfield Mission will provide a free advice and support service for a wide range of people with particular focus on supporting those who have mental health issues, are homeless or are vulnerable in some other way.
Dementia Friendly Keighley (DFK) – Bradford

An 18-month project to support people living with dementia in Keighley, Bradford. The DFK project will provide a referral pathway to support those living with dementia, their carers/families during their dementia journey and to ensure all families affected by dementia can access both medical and community services in the Keighley locality.
The Candlelighters Trust – Bradford, Huddersfield, Halifax, Leeds and Wakefield

A one-year project to deliver a community-based support model ‘Candlelighters in Your Community’ (CIYC) aimed at supporting families affected by childhood cancer across Yorkshire. The Candlelighters Trust project will fund two Community Support Workers who will deliver Candlelighters In Your Community, a service which will support families affected by childhood cancer within their local communities.
The Art House – Wakefield

A one-year project that will focus on supporting new mothers, pregnant mothers, those with dementia and their careers, asylum seekers and refugees and those leaving prison. The ART House project through developing and delivering a series of group programmes and activities combining arts, the great outdoors and mindfulness practices, the ‘Re-emerging’ project aims to help vulnerable people whose mental health has particularly suffered due to the pandemic to re-engage with others in their community so that they can successfully resume their daily lives in a post-pandemic world.
North of England Activities and Training (NEAT) - Bradford

A one-year project to deliver outdoor activities for ethnic minority women and deaf people in Bradford.
The NEAT project will increase the provision of strenuous outdoor exercise and ecotherapy to minority groups, including people who are deaf, through training new leaders and overcoming transport barriers.
Barca-Leeds – Leeds
Amount funded - £100,092
A one-year health and wellbeing navigators pilot project in Leeds for people sleeping rough, homeless women, asylum seekers and refugees and people with no recourse to public funds, to access the healthcare they need. Barca Leeds through the project using Health Navigators will provide intensive one-to-one support into health services to address the complex needs of those people sleeping rough. The pandemic highlighted the need for improved access routes into health for people who are rough sleeping.
Rugby League Cares – Leeds, Bradford, Halifax and Huddersfield
Amount funded - £98,907
An 18-month project to work with men, disproportionately impacted by Covid-19, and therefore at increased risk of poor mental health and suicide within Leeds, Featherstone, Dewsbury, Hunslet, Batley and Keighley. Rugby League Cares (RLC), through the Offload project wants to deliver a mental fitness programme for men. Delivered by rugby league players, it gives men tools to develop a positive mindset and recognise when they or others require further support.
Leeds GATE (Gypsy and Traveller Exchange) – Bradford and Wakefield
Amount funded - £74,848
An 18-month project to deliver a project for Gypsies and Travellers across the Wakefield and Bradford Districts, working on local authority run sites and roadside encampments.
Leeds GATE will work in an assist-based community development approach to reduce health inequalities for Gypsies and Travellers living in West Yorkshire.
Keighley Healthy Living (KHL) – Bradford and Keighley
Amount funded - £75,000
An 18-month project with a focus on vulnerable community members identified as being disproportionately affected by Covid-19 who live in Keighley. The target groups include South Asian women (including those living with mental health conditions); unpaid carers; older people aged 50 and over; BAME groups and people living with learning disabilities and who have additional needs.
KHL through the project wants to co-design and test-and-learn from the development of a ‘clinical to community health care pathway’ to-simplify and connect the ‘user journey’ from clinical-health-services to the voluntary sector community-health and crisis support offer, to support people’s longer-term-health solutions as part of their self-care.
