Our Bradford District and Craven Health and Care Partnership is supported by an 'Act as One' ethos which means we work together so that we can achieve our vision of people living ‘happy, healthy at home.’
To help with this, we have redefined our place based strategy in November 2021 which sets out how we will work together to deliver on our ambitions. These ambitions will contribute to our efforts to tackle inequalities by taking a population health approach that is rooted in intelligence and knowledge of our communities.
Our approach is based on our firmly held belief that we give power to our people and our communities so that they can help determine our decision making. However there are other times where we need to make decisions at scale either as a place (Bradford District and Craven) or as part of our wider system (West Yorkshire Health and Care Partnership). This is what we mean by inverting the power to act and it is one of our key principles we use to ensure everyone has the opportunity to contribute to achieving our vision.
Our four core purposes are:
- Improving population health and wellbeing
- Tackling unequal outcomes and access to care
- Ensuring a sustainable health, care, and wellbeing system
- Supporting broader social and economic development.
Our Act as One approach is one we are proud of and we can already demonstrate how this is making a real difference to the lives of our communities as well as our colleagues. In May 2021, during our Act as One Festival, we held a recognition day to celebrate individuals, teams and projects that are living the values of Act as One.
